A fully customizable, one-page form to protect your salon's equipment before it walks out the door — used across Anya's own three salon locations. Covers property issued (item, serial number, condition, value), a seven-point acknowledgment of responsibility and recovery terms, signatures, and a return log for when the item comes back at resignation, termination, or role change.
Issue it the same day you hand over any equipment, uniform, keys, or devices — not weeks later once it's already been used or damaged.
